If difficulties with the form please notify the Aircraft Manager 

A standard flight report needs to be submitted to the Aircraft Manager upon return of DMP declaring the flying activities that took place during the period of hire;

-        Keep a record for yourself and report the following;

-        VDO OUT and VDO IN

-        Tacho OUT and Tacho IN 

-        Air Switch OUT and Air Switch IN 

-        Please document the number of landings at designated ICAO (or private airstrip); e.g. YMYB → YBSU → YHBA → YMYB; x3 touch and go @ YBSU + x1 Missed Approach @ YHBA, Full stop @ YHBA

-        as long as makes sense I don’t mind how you do this- Matt Cozier -

-        Report any concerns or incidental notes; e.g. “oil down to 3 qts, topped up”, or “keys left in ignition”, or “door left unlocked” … or “fuel not topped up at start of hire”;

-        Images are preferred to be uploaded for further referencing and problem-solving if errors in the record arise.

Please note that the flight report form DOES NOT replace the maintenance release!
